School Supplies Form

Forms must be submitted no later than Aug. 4. As a token of our appreciation for all parents who submit their forms by Aug. 1, we will include a new pair of socks and underwear for each child. You can also print this form, and mail it to: MFCAA/School Supplies/3210 S Lee's Summit Rd/Independence, MO 64055.

If a placement changes and you need to remove a child or add a child for school supplies, please let us know as soon as possible.
Supplies will be bagged up according to the information you provided above. All you have to do is come to MFCAA to pick up the bags for your children... it should take no longer than 5 or 10 minutes (if you do not submit a form prior to Aug. 4, please be prepared to wait longer as we will have to bag your supplies when you get there).

*This is not an appointment – should your plans change, you are not required to come during the time you choose here. We would just like to get an idea of the number of people that plan to come during each pickup period, so that we can schedule the correct number of volunteers.

If you have more than eight children, please fill out this form and submit it, then fill out an additional form for the additional children. *We will make every effort to honor your underwear preference, but due to supply numbers we cannot guarantee the underwear item that your child receives.
